Who We Are

A nonprofit organization, IAC Vascular Interventional is supported by the following organizations while operating independently of their activities. Representatives from these organizations, listed below, make up the IAC Vascular Interventional Board of Directors.

American College of Surgeons (ACS)

Kathleen Ozsvath, MD, FACS serves on the IAC Vascular Interventional Board of Directors as a representative of the American College of Surgeons (ACS). Dr. Ozsvath is Professor of Surgery at Albany Medical Center in Albany, NY, Chief of Surgery at Samaritan Hospital in Troy, NY. Dr. Ozsvath earned her medical degree at the University of Texas Southwestern Medical School. She completed her general surgery internship and residency at St. Luke’s Roosevelt Hospital, College of Physicians and Surgeons in New York, and vascular surgery fellowship at Albany Medical Center in Albany. She serves as a faculty member for the American Venous Forum (AVF) as member of the Fellows Education Committee, is the past chairperson of the AVF Research Committee, and present chairperson of the AVF Health Policy Committee. She also serves on several committees for the Society of Vascular Surgery (SVS). Dr. Ozsvath is a published author whose work is found in many respected journals such as the Annals of Vascular Surgery and the Journal of Vascular Surgery. She is also an accomplished speaker and has presented internationally.

American Vein and Lymphatic Society (AVLS)

Khanh Q. Nguyen, DO, RPVI serves on the IAC Vascular Interventional Board of Directors as a representative of the American Vein and Lymphatic Society (AVLS). Dr. Nguyen holds active certifications by the American Board of Osteopathic Emergency Physicians, the American Board of Venous and Lymphatic Medicine, and is a Registered Physician in Vascular Interpretation. Dr. Nguyen completed his medical training at Kirksville College of Osteopathic Medicine, and residency at Botsford General Hospital, part of the Michigan State Health System. He has received extensive training in the treatment of venous disease and is also involved in training the next generation of venous experts. Dr. Nguyen serves as the Designated Institutional Officer for one of AVLS’s first Phlebology Fellowship Programs. Currently, he is the Chief Medical Officer for the Centers for Vein Restoration, the largest physician lead vein practice in the country with over 70 centers nation wide.

Carl Fastabend, MD, FACC, FSCAI, RPVI serves on the IAC Vascular Interventional Board of Directors as a representative of the American Vein and Lymphatic Society (AVLS). Dr. Fastabend earned his medical degree from the Medical College of Virginia, where he also completed his residency in Internal Medicine and Cardiology. He is board certified in Internal Medicine and Cardiovascular Disease and is a diplomate of the American Board of Phlebology, and has the Registered Physician in Vascular Interpretation certification. He is active in the AVLS, serving on the Health Advocacy Committee, and is a member of the American Venous Forum (AVF). Dr. Fastabend founded the Vein Center of Southwest Louisiana in 2011 and has developed a comprehensive vein practice with a special interest in deep venous disease. He is involved in research as well as educating physicians in vein disease and is a frequent speaker at major venous symposia.

American Venous Forum (AVF)

Fedor Lurie, MD, PhD, RPVI, RVT serves on the IAC Vascular Interventional Board of Directors as a representative of the American Venous Forum (AVF). Dr. Lurie is the Associate Director of Jobst Vascular Institute of Promedica in Toledo, OH, and adjunct research Professor of Vascular Surgery at the University of Michigan, Ann Arbor, MI. Dr. Lurie earned his medical degree at the Urals Medical Academy in 1980, where he also completed his general surgery residency and fellowship in vascular surgery. Dr. Lurie’s clinical and research interests include acute deep venous thrombosis, chronic venous disease and diagnostics of vascular disorders. He is a past president of the AVF and a member of the Society for Vascular Surgery (SVS), the American Vein and Lymphatic Society (AVLS) an honorary member of the European Venous Forum (EVF) and several international venous societies. He serves on the AVF Health Policy Committee, AVF CEAP Task Force and AVF Guidelines Committee. He also serves on several committees for SVS. Dr. Lurie is a published author whose work is found in many respected journals such as the Annals of Surgery, European Journal of Vascular and Endovascular Surgery, Phlebology, Journal of the American Medical Association, Thrombosis Research and the Journal of Vascular Surgery. He is also an accomplished speaker and has presented internationally.

Mark Meissner, MD serves on the IAC Vascular Interventional Board of Directors as a representative of the American Venous Forum (AVF). Dr. Meissner is a professor in the Department of Surgery at the University of Washington. Dr. Meisser earned his medical degree from the University of Colorado School of Medicine in 1985. He then completed his residency, his fellowship in Vascular Surgery and his fellowship in Interventional Radiology at the University of Washington. Dr. Meisser’s clinical interests are acute deep venous thrombosis, chronic venous disease and the management of pelvic venous reflux. He is a past president of the AVF and a member of the Society for Vascular Surgery (SVS), the American College of Surgeons (ACS) and the American Vein and Lymphatic Society (AVLS).

Society for Cardiovascular Angiography and Interventions (SCAI)

Kenneth Rosenfield, MD, MHCDS serves on the IAC Vascular Interventional Board of Directors as a representative of the Society for Cardiovascular Angiography and Interventions (SCAI).

Society for Clinical Vascular Surgery (SCVS)

Joann Lohr, MD, FACS, RVT, CWSP serves on the IAC Vascular Interventional Board of Directors as a representative of the Society for Clinical Vascular Surgery (SCVS).

Colleen J. Moore, MD, FACS, RPVI, MHM serves on the IAC Vascular Interventional Board of Directors as a representative of the Society for Clinical Vascular Surgery (SCVS). Dr. Moore is a vascular surgeon with Prairie Cardiovascular Consultants in Springfield, IL. She earned her medical degree from the University of Missouri-Kansas City School of Medicine. Dr. Moore completed two preliminary years in general surgery followed by two categorical years at the Conemaugh Hospital in Johnstown, PA before transferring to the University of Missouri Hospitals and Clinics in Columbia, MO to complete her training. She completed a two-year vascular surgery fellowship at the University of Missouri Hospitals and Clinics. Dr. Moore is a member of the American Venous Forum (AVF) where she was the program chair and currently serves on both the membership and newsletter committees. She is also a member of the Society for Vascular Surgery (SVS) and serves on their coding committee. She has volunteered with the Inteleos corporation to lead the physician’s vascular interpretation examination and now serves on the Inteleos Board of Directors. She also chairs the technology and innovation committee.

Society for Vascular Medicine (SVM)

Thom W. Rooke, MD, RVT serves on the IAC Vascular Interventional Board of Directors as a representative of the Society for Vascular Medicine (SVM). Dr. Rooke obtained a BS in Physiology at the University of Michigan and his medical degree at Johns Hopkins School of Medicine. Upon completing residencies in Internal Medicine and Cardiovascular Medicine at Mayo Clinic in Rochester, N.Y., he joined the Vascular Medicine section at Mayo, where he now serves as the Krehbiel Professor of Vascular Medicine. Dr. Rooke also founded the Mayo Clinic Wound Care Center. He serves on a number of national boards, committees and societies. In addition to having served as President of IAC Vascular Testing, Dr. Rooke is also the past-president of the SVM, where he has a long history of service. His interests include medical writing — fact and fiction, peripheral vascular medicine, varicose veins, swollen limbs and the art of the medical lecture.

Society for Vascular Nursing (SVN)

Katherine Hays, DNP, ACNP-BC, FSV serves on the IAC Vascular Interventional Board of Directors as a representative of the Society for Vascular Nursing (SVN). She is currently working as a vascular medicine nurse practitioner at OhioHealth Heart and Vascular. She received her Master’s degree in Nursing from Wright State University and her Doctor of Nursing Practice from Otterbein University in Dayton, OH. In addition to SVN, she is a member of the Society of Vascular Medicine (SVM), American Vein and Lymphatics Society (AVLS), American Venous Forum (AVF) and the Ohio Association of Advanced Practice Nurses (OAAPN). She serves on the SVN Task Force for the society’s annual conference and is a member of the Membership Committee, the Scientific Sessions Committee and the Fellows/APP Course Planning Committee for SVM.

Society for Vascular Ultrasound (SVU)

Sara Skjonsberg BS, RVT, RPhS serves on the IAC Vascular Interventional Board of Directors as a representative of the Society of Vascular Ultrasound (SVU). She graduated from Seattle University's Diagnostic Ultrasound Program in 2013. She worked for the University of Washington DE Strandness Jr. Vascular Lab for four years where she received extensive venous training with Dr. Mark Meissner who jump started her involvement with the American Vein and Lymphatic Society (AVLS), SVU and American Venous Forum (AVF). She is currently the program chair for the ultrasound section at that AVLS Annual Congress and adjunct faculty member at Bellevue College. She is currently working as a staff sonographer for Oregon Vascular Specialists in Bend, OR with Dr. Wayne Nelson and Dr. Jason Jundt.

Society of Interventional Radiology (SIR)

Domenic A. Zambuto, MD, RPVI serves on the IAC Vascular Interventional Board of Directors as a representative of the Society of Interventional Radiology (SIR). Currently, Dr. Zambuto is a physician and senior medical director with Vein Clinics of America. He is board-certified in vascular and interventional radiology/diagnostic radiology from the American Board of Radiology. He is also an ABVLM diplomate. Prior to joining Vein Clinics of America, he served as Director of Interventional Radiology at Hartford Hospital. Dr. Zambuto received his medical degree from Harvard Medical School and has completed both his residency in diagnostic radiology and a fellowship in vascular interventional radiology at Massachusetts General Hospital. He was Chief Resident in the Department of Radiology and Chief Fellow in Magnetic Resonance Imaging at Massachusetts General Hospital. In addition to SIR, he is an active member of the American College of Phlebology (ACP).

Ramona Gupta, MD, FSIR serves on the IAC Vascular Interventional Board of Directors as a representative of the Society of Interventional Radiology (SIR). Dr. Gupta currently works at Northwestern Memorial Hospital and serves as chief of interventional radiology at the Jesse Brown VA Hospital in Chicago. She received her bachelor’s from Dartmouth College in Hanover, NH and her medical degree from the University of Arkansas for Medical Sciences in Little Rock, AR. After completing her diagnostic radiology residency at Emory University in Atlanta she completed her vascular and interventional radiology fellowship at Northwestern. Dr. Gupta has 15 years of experience and is certified by the American Board of Radiology (ABR) in interventional and diagnostic radiology. She serves on the Venous Clinical Specialty Council for SIR and her clinical interests include superficial venous disease, chronic venous disease and venous ulcers and pelvic venous disease.

Derek Mittleider, MD, FSIR, RPVI serves on the IAC Vascular Interventional Board of Directors as a representative of the Society of Interventional Radiology (SIR). Dr. Mittleider currently works with Brevard Physician Associates and as the Chief of Vascular and Interventional Radiology at the Holmes Regional Medical Center in Melbourne, FL. He received his B.S. and B.A. degrees from the University of New Orleans and afterwards obtained his M.S.P.H. from Tulane University School of Health and Tropical Medicine. Dr. Mittleider earned his medical degree from Louisiana State University Medical School in 2004. He conducted his post graduate training at the Maine Medical Center and the Miami Cardiac and Vascular Institute. Dr. Mittleider is certified by the American Board of Radiology (ABR) in Interventional Radiology and Diagnostic Radiology and by the Alliance for Physician Certification & Advancement (APCA) as a Registered Physician in Vascular Interpretation. He currently serves on several SIR committees including the executive, wellness and burnout and the standards committees. He also serves on the arterial workgroup, the venous workgroup, and the IR advisory group to the RRC. Dr. Mittleider is a published author whose work is found in many respected journals such as the American Journal of Interventional Radiology, the Journal of Vascular and Interventional Radiology and Techniques in Vascular and Interventional Radiology. He is also an accomplished speaker and presenter and has presented at many professional society meetings.

Members-at-Large

Andrey Espinoza, MD, FACC, FSCAI, RPVI serves on the IAC Vascular Interventional Board of Directors as a Member-At-Large. Dr. Espinoza is currently an interventional cardiologist and endovascular specialist at the Advanced Heart and Vascular Institute of Hunterdon (AHVI) in Flemington, NJ. He is currently the director of the AHVI Research Division and the medical director of the Atlantic Vascular Center in Raritan, NJ. He received his medical degree from the Saba University School of Medicine and went on to complete his internship, residency and fellowships at the Medical College of Pennsylvania/Hahnemann University Hospitals and the Drexel University College of Medicine in Philadelphia. He participates in numerous professional societies including the American College of Cardiology (ACC), the Society for Cardiovascular Angiography and Interventions (SCAI), and the American Medical Association (AMA). Dr. Espinoza also serves on the SCAI Carotid Committee, as a member of the Vascular Disease Council, as a manuscript reviewer for Cardiac Catheterization and Interventions and as an editorial advisory board member for Cardiac Interventions Today. He has given numerous presentations at professional society conferences including SCAI and Transcatheter Cardiovascular Therapeutics (TCT) and he has authored and co-authored multiple papers in professional journals including American Journal of Cardiology, Circulation and Catheterization and Cardiovascular Interventions. Dr. Espinoza had made many contributions to the research field by participating in over 50 clinical studies that have led to the development of both FDA approved pharmaceuticals and contemporary technology for the coronary and vascular space.

Arjun Jayaraj, MD serves on the IAC Vascular Interventional Board of Directors as a Member-At-Large. Dr. Jayaraj is a Vascular Surgeon at the RANE Center for Venous and Lymphatic Diseases at St. Dominic Hospital in Jackson, MS. He received his Bachelor of Medicine / Bachelor of Surgery degree from Calicut Medical College, Kozhikode in Kerala, India and completed his surgical residency at the University of Washington in Seattle. He then went on to train in vascular surgery at the Mayo Clinic in Rochester. He is currently board certified by the American Board of Surgery in the areas of general surgery and vascular surgery. Besides authoring/coauthoring several original research articles, Dr. Jayaraj sits on the editorial board for Veins and Lymphatics. Additionally, he has also been a presenter and moderator/panelist for several prestigious conferences. He is a member of the American College of Phlebology (ACP) Guidelines Committee in addition to being part of the American Venous Forum (AVF) Writing Group.

Margaret Mann, MD, FAAD, FACPh serves on the IAC Vascular Interventional Board of Director as a Member-At-Large. Dr. Mann is a board-certified dermatologist with fellowship training in Mohs surgery, dermatologic surgery, cosmetic surgery and vein surgery. Dr. Mann received her medical degree and dermatology residency training at Washington University School of Medicine in St. Louis, MO. She then completed a fellowship in Mohs surgery and Cosmetic Dermatologic Surgery at the Center of Dermatologic and Cosmetic Surgery at Washington University, where she received extensive training in the treatment of venous disease. She is currently the Director of Aesthetic Dermatology at University Hospitals Case Medical Center as well as the Associate Director of Dermatologic Surgery and an Associate Professor of Dermatology at Case Western Reserve School of Medicine. Prior to joining University Hospitals, Dr. Mann was the Chief of Clinical Services in the Department of Dermatology at the University of California, Irvine. She was also the Co-director of Dermatologic and Cosmetic Surgery and the Mohs Surgery Fellowship Program at UC Irvine. She serves on the Board of Directors of the American College of Phlebology (ACP) and, in the past, served on the Board of Directors of the California Society Dermatology and Dermatologic Surgery. Additionally, Dr. Mann is an associate editor for the British Journal of Dermatology.

Gilly Munavalli, MD, MHS, FAAD, FACMS serves on the IAC Vascular Interventional Board of Directors as a Member-At-Large. Dr. Munavalli is the medical director and founder of Dermatology, Laser & Vein Specialists of the Carolinas in Charlotte, NC. He trained in dermatology at Emory University and completed dermatologic surgery fellowships in Mohs Micrographic Surgery, cutaneous oncology, facial reconstruction, cosmetic surgery and laser surgery at the University of California-San Francisco. Dr. Munavalli also completed a phlebology and advanced laser surgery fellowship at the Maryland Laser, Skin and Vein Institute and Johns Hopkins University. He received his Bachelor of Science from the University of North Carolina-Chapel Hill, Masters of Health Science from the Johns Hopkins University School of Public Health, Doctorate of Medicine from Morehouse Medical School, and internship training at the Mayo Clinic. Dr. Munavalli received his Board Certification from the American Board of Dermatology in 2002. Dr. Munavalli is a fellow member of the American Academy of Dermatology (AAD), the American College of Mohs Surgery (ACMS), the American Society for Laser Medicine and Surgery (ASLMS) and the American Society for Dermatologic Surgery (ASDS). Additionally, he is a member of the American College of Phlebology (ACP). Dr. Munavalli also serves as a Cosmetic Dermatologic Surgery Fellowship Director for the ASDS. He is currently affiliated as an Assistant Professor of Dermatology with the Wake Forest University School of Medicine - Department of Dermatology. Previously, he spent four years on the clinical faculty of Dermatology at Johns Hopkins School of Medicine, in Baltimore, MD.

Deepak Nair, MD, FACS, MS, MHA, RVT serves on the IAC Vascular Interventional Board of Directors as a Member-At-Large. Dr. Nair currently practices in Sarasota, Fla. An educator and mentor, he is also a Clinical Assistant Professor in the Department of Clinical Sciences (Vascular Surgery) at Florida State University. Dr. Nair earned his medical degree from Upstate Medical Center in Syracuse, N.Y., completed his medical internship and residency at Albert Einstein College of Medicine and completed his fellowship at Emory University School of Medicine. He is board certified in vascular surgery and is a diplomat of the American Board of Surgery (ABS). Dr. Nair has received a plethora of awards and honors ranging from the Golden Apple Award for Best Resident Teacher in 2005 and 2006 to a Patient Care Trust Fund Grant in 2003. He is currently a member of the American Venous Forum (AVF), the Sarasota Medical Society (SCMS), the Florida Medical Association (FMA), the Society for Vascular Ultrasound (SVU) and the American College of Surgeons (ACS).

CEO

Mary Lally, MS, CAE is the Chief Executive Officer at the Intersocietal Accreditation Commission (IAC). Ms. Lally began her tenure with the IAC in 2007, serving as Director of Accreditation. In this role she has been instrumental in the development and oversight of the IAC’s MRI and CT diagnostic divisions as well as the Carotid Stenting, Cardiac Electrophysiology and Cardiovascular Catheterization therapeutic divisions. Ms. Lally has a unique combined background of clinical expertise and association management experience, possessing both a Master’s degree in Radiation Health Physics as well as the Certified Association Executive (CAE) credential. Before joining the IAC, Ms. Lally gained more than 20 years of clinical, operational and leadership experience, serving as the founding MRI program director at the Johns Hopkins Hospital. In addition, she developed and implemented radiology quality assurance programs with emphasis in the vascular surgery, interventional and cardiology departments at the University of Maryland Hospital. An ARRT-certified MRI Technologist, Ms. Lally is a passionate advocate for quality and safety in imaging.

Director of Accreditation – Vascular Interventional

Laura Humphries, BS, RVT, RPhS joined the IAC in December 2013 and serves as the Director of Accreditation for IAC Vascular Interventional and Carotid Stenting. Her experience includes more than 20 years in the field of peripheral vascular ultrasound. Mrs. Humphries earned her Bachelor of Science degree from Oregon Institute of Technology in Klamath Falls, WA. She is credentialed by the American Registry of Diagnostic Medical Sonography (ARDMS) in Vascular Technology and by Cardiovascular Credentialing International (CCI) as a Registered Phlebology Sonographer. She is an active member of the ARDMS, Society of Vascular Ultrasound (SVU) and CCI.
