The facility procedure log is the list of all patients who have been to the vein center for evaluation or a treatment. This is part of the application and is uploaded into our online questionnaire. The procedure complication log is an individual log kept for each operator to document the outcomes of the procedures that they have performed on individual patients. The procedure complication logs will be reviewed during a site visit.
