An online account is comprised of two aspects: the Account Profile and the application questionnaire(s). Listed below is a general overview of completing an online application.

  • Log in to your account and fill out the required information (indicated by bold type and a red asterisk *) in the Account Profile.
  • On the Applications tab, access the questionnaire which contains specific questions relating to the operations of your facility some of which require evidence of supporting documentation (read more about Uploading Attachments).
  • Submit your completed application using the [Submit] button that will appear at the conclusion of the questionnaire. The submitted application questionnaire becomes your final application for the given cycle and you will be unable to make changes to the locked questionnaire. However, you will be able to review your application at any time by logging in to the account and clicking the appropriate icon on the Applications tab.
  • If your facility is planning to apply for IAC accreditation in another modality, you may use the same account. The universal application will enable your facility to complete and submit applications for those additional types of accreditation, utilizing the same facility information that you have customized for your previously entered applications.
  • Facilities may view and download their application decision letters and Application Review Findings (ARF), as well as other written correspondence, through the online portal. The download capability is available for facilities with applications currently in progress and applications submitted from May 15, 2012 forward.
