At this time, there is not a questionnaire in the Online Accreditation application to complete when adding a multiple site (fixed and/or mobile) to an already accredited facility.

Accredited facilities wanting to add a multiple site (fixed and/or mobile) must submit the completed Multiple Site (Fixed and/or Mobile) Supplemental Application via trackable mail. If there are any physicians and/or technologists at the additional site that were not included as part of the original application, it is necessary to complete the necessary staff forms located in the Changes Within the Facility section. ALL of the forms included in the Multiple Site (Fixed and/or Mobile) Supplemental Application, available for download above, including the Affidavit of Change in Ownership or Operations and the necessary staff forms must be submitted. Sites are not considered accredited until the documents have been reviewed, fees have been submitted and accreditation has been granted.

It is critical that you access your facility's IAC Online Accreditation account as soon as possible to enter and/or verify the new site and update the National Provider Identifier (NPI) data and Medicare enrollment numbers. CMS as well as other private insurers will verify the accreditation status of a supplier only through data file transfers containing this information. Failure to enter accurate identification data will result in nonpayment of Medicare claims after 1/1/2012. To view the full instructions of updating the NPI data or Medicare enrollment numbers, click here.

Upon receipt of all required materials, the accreditation decision will be made at the next monthly IAC Board of Director's meeting. If the added multiple site (fixed and/or mobile) is accepted into the facility's accreditation, accreditation for the added multiple site (fixed and/or mobile) will expire when the facility's original accreditation expires.

Effective 8/10/2011: On March 28, 2011 the IAC announced a policy change related to mobile services. At that time, the previous mobile service policy was eliminated and the current multiple site (fixed and/or mobile) policy was placed into effect. After review of the fees associated with mobile vascular and echocardiography multiple site applications the following change in fees will be effective immediately: $750 per site for sites 2-10; $400 per site for all sites over 10; base fees and testing area fees still apply. The updated policy is published in the IAC Accreditation Program Policies and Procedures, Section 2, on page 5.
