All facilities are required to submit the items listed below as part of the Online Accreditation application:

• Equipment Information (manufacturer, model, year)
• Procedure Volumes (annual staff and facility procedure volume information) | sample procedure volumes»
• Training/Experience Qualification Pathways for Physicians and Sonographers (i.e., Level II or Level III training for physicians; RCS, RDCS for sonographers)
• Certificate/Credential Information (i.e., ABIM certificate(s) for physicians, RDCS, RCS for sonographers [including dates and certificate/registry numbers])
• An Estimate of the Number of Studies Interpreted by the Medical Director and Every Medical Staff Member
• CME Information for All Interpreting Physicians and Technologists
  Specific course/lecture title, date of attendance, type and number of credits earned. All staff members are required to submit 15 CME relevant to echocardiography every three years; even if they are new to the facility. sample CME log»
