As your facility prepares to submit the Online Accreditation application, it’s important that you review your application for accuracy and ensure that you are submitting all of the necessary items.The IAC suggests applicant CT facilities hold a Quality Improvement meeting prior to submission to review the entire application for accuracy and completeness.

Here’s a brief summary of what you should have ready to submit:


Quick Checklist

Equipment Information – Did you submit all of the necessary equipment information (i.e., serial number, software version, date of installation, field strength)?

Training/Experience Pathways for Physicians and Technologists – Is your staff fulfilling all of the necessary Training/Experience Pathways as required by the IAC Standards and Guidelines for CT Accreditation?

Procedure Volumes – Did you submit all of the necessary annual staff and facility procedure volume information?

Certificate/Credential Information – Did you submit all of your staff’s necessary certificate/credential information as required by the IAC Standards and Guidelines for CT Accreditation? (i.e., ABMS or board certification for physicians; (RT (CT). CNMT. RT) for techologists)

CME Information for ALL Staff Members – Did you enter the required amount of CE/CME into the Online Accreditation application for each staff member?

Required Documentation – Did you submit all of the required policies and/or protocols required?

Once you hit send on your IAC Online Application, it is imported into the IAC database.
Facilities have five days to submit the following via traceable postal mail to
IAC | CT, 6021 University Blvd., Suite 500, Ellicott City, MD 21043:

Case Studies (in duplicate) and Corresponding Final Reports - Have you properly prepared the necessary case studies for submission?

Quality Improvement Documentation - Did you submit all necessary QI documentation?

IAC Accreditation Agreement - Is your Agreement filled out completely, signed and ready to submit?

Signed Attestation Pages - Do you have signed attestation pages for all staff members? (e.g., Medical Director, Technical Director and medical and technical staff)

Fees (if paid by check) - Do you have your complete payment ready? (i.e., Accreditation Fee)
