Changes made within an accredited facility will require specific changes to the documentation for the facility, most notably the facility's IAC Accreditation Agreement. The facility must notify the IAC within 30 days of the effective date of change.

If Your Facility is Experiencing Any of the Following Changes:

CHANGE IN NAME OR CHANGE IN OWNERSHIP

The facility is required to notify the IAC and submit the form, Affidavit Approving Change In Ownership / Operations, available on the IAC website, or similar formal legal documentation. For complete instructions and to download the sample affidavit, click here to visit the Legal section of the IAC website and scroll down to Change In Name or Change In Ownership.

CHANGE IN OPERATIONS (i.e., Medical Director, Technical Director, Permanent Site Location or Mobile Service)

The facility is required to notify the IAC and submit the form Affidavit Approving Change In Ownership / Operations, available on the IAC website, or similar formal legal documentation. When a facility is requesting to a change in Medical or Technical Director, but is not changing the rest of the staff, the following is required:

  • The new Medical or Technical Director must meet the IAC CT Standards.
  • The Affidavit Approving Change In Ownership/Operations
  • If the current Medical or Technical Director was not included in the original application, the Medical Director Form or Technical Director Form must be completed and submitted to the IAC CT along with a copy of the current license, ARRT card or other credentials, all requested documentation and 15 CT specific CE/CME for the last three years. An attestation is required as well and can be found at the end of the Medical or Technical Director forms listed above.

Please note: If the new Medical or Technical Director was already listed in the application as staff and meets all IAC CT Standards, only the formal letter must be submitted to the IAC CT.

CHANGE IN MEDICAL OR TECHNICAL STAFF

When a facility is requesting to a change in medical or technical staff, the following is required:

  • All new staff members must meet the IAC CT Standards.
  • A formal letter signed by the Medical or Technical Director requesting that the IAC CT add the new staff members to the application. The letter must include all new staff member names.
  • If any current medical or technical staff were not included in the original application, the Medical Staff Form or Technical Staff Form must be completed and submitted to the IAC CT along with a copy of the current license, ARRT card or other credentials, all requested documentation and 15 CT specific CE/CME for the last three years. An attestation is required as well and can be found at the end of the Medical or Technical Staff forms available for download above.

CHANGE IN EQUIPMENT

When a facility is replacing or adding an additional CT unit, the following is required:

  • A formal letter of notification from the Medical or Technical Director that the facility has replaced or added an CT system.
  • Documentation of physicist acceptance testing performed, prior to clinical use to include dose assessment, shielding verification and QC tests.
  • 5 days of daily QA tests (with test results and corresponding phantom images) performed since unit installed and most recent PM report
  • 1 case study and 1 final report (representative of the testing area).

  • Submit the CT Scan Parameter Form
  • Submit the CT Replacement or Adding a Unit Form
  • $800 fee for adding a unit.
